Who Needs A Smart Health Certificate?

You need a Smart Health Certificate if you have:

• No health insurance
• High deductible
• Insurance that most physician offices do not accept

What Is The Price Of A Smart Health Certificate?

Osprey PolyClinic offers 2 levels of Smart Health Certificates:

• Essential Level: $500 Certificate valued at $1,000
• Premium Level: $1,000 Certificate valued at $2,500

How Do I Use A Smart Health Certificate?

Smart Health Certificate qualifies you to have a HIPAA compliant account with Osprey PolyClinic. Your account will be funded with the Certificate’s value. Anytime you receive a service, your account balance will be debited at the self-pay rate for that service.

Example: Cost of Annual Wellness Exam is $225. When you come in for an Annual Wellness Exam, your account will be debited $225.

What Types Of Services Are Included With Smart Health Certificate?

All available health care lab and imaging services provided at Osprey PolyClinic.

What Type Of Payment Do You Accept?

Smart Health Certificates can be purchased with cash, debit, credit card, check or Health Savings Account (HSA) card.

Yes! Osprey PolyClinic offers flexible financing with CareCredit.

CareCredit is a convenient, flexible way to pay for health care services. With everyday promotional financing for purchases of $1,000 or more, you can pay as little as $31/month.

• Smart Health Certificate is not an insurance or an insurance replacement product.
• Smart Health Certificate is not refundable or transferrable.  
• Valid only at Osprey PolyClinic for 12 months from the date of purchase.
• Smart Health Certificate holder is responsible for remaining balance if Certificate value is depleted.  
• (1) Labs include CBC, Comp Metabolic Panel, Lipid Panel, TSH W/ Reflex To FT4, Hemoglobin A1C, Urinalysis Complete, B12 & Folate. Additional labs are available at 20% discount of self-pay rates.
• (2) All diagnostic services require an order from Osprey PolyClinic’s Physician.
• (3) Membership program require subscription.  Certificate value can be applied to 1st month only.
